True Patient Centeredness
Patient care begins with the individual, and the individual is lost when all patients are treated the same. "One-size-fits-all" is not patient centered care. Communication that ignores individual differences in learning styles, motivation and preparedness lose both efficiency and effectiveness in creating desired outcomes.
The PATH system advances patient centered care at two levels:
First, the focus is squarely on the individual-the patient receiving care-defined in terms of psycho-social factors that shape perceptions, predict behavior, and influence outcomes.
Second, the PATH model predicts disease prevalence and utilization. The PATH model is therefore ideal for targeting specific service lines to high need segments, and adds the ability to tailor offerings and messages to them based on known patterns of healthcare preferences and priorities. The result is more effective community outreach. |
